Your Visit To Our Office…

We want to ensure your visit is as pleasant as possible. To help make your visit go smoothly you can download and complete our New Patient Forms before arriving at our offices.

We accept most major insurances and have special programs for patients without insurance. We also special financing programs thru CareCredit – click to apply.

Additionally, here are our safety protocols for your visit:

* Text the office at 813-955-0268 upon arrival (name & appointment time). We will text you back when we are ready for you to enter the office.

* Only the patient receiving treatment are permitted in the office and/or operatory. For minor children or special needs patients we will allow 1 person in the office.

* Screening questions, prior to and day of, as well as temperature checks upon arrival are mandatory at each visit

* Office hours are: Monday – Thursday 7am to 5pm (closed 12-1 for lunch).
